This interdisciplinary initiative investigates barriers and challenges identified by parents of adolescent patients with complex medical needs. Our goal is to design digital resources supporting these parents as their adolescents transition from pediatric to adult care. In collaboration with the Stollery Children's Hospital Pediatric Cardiology Clinic and Adolescent Medicine Clinic, we will bring together parents, medical practitioners, information experts, designers, and students to develop solutions for parent-identified barriers and challenges.
